We are a registered non-profit organization providing supportive housing programs. Our main services include:
Life Skills Programming: Transitions Life Skills Bootcamp offers eight essential skills for true independence, such as financial literacy, savvy shopping and cooking, post-secondary life, tenancy rights and responsibilities, whole health, healthy relationships and sexual health, job skills and career planning, and building self-esteem and goal planning.
Case Management/Mentoring: Personalized support and guidance to help youth navigate their challenges and achieve their goals.
Affordable Housing: We aim to provide subsidized housing support, though availability depends on funding.
Founded by a former youth in care, a master’s level registered social worker, former foster parent, youth advocate, and mother of three, Transitions Supportive Housing Programs was established to address the ongoing challenges faced by youth transitioning from care. Transitions is dedicated to improving outcomes for at-risk youth. Our ultimate goal is to become a youth-led organization.

As a non-profit, some services, including subsidized housing support, are dependent on generous funders.
No, any youth considered at risk can be referred. This includes youth who are precariously housed, part of minority populations, have issues with the law, or face challenges at school, among others.
Youth can self-refer to our programs.
We typically run programs in the fall, spring, and summer. Our Life Skills Bootcamp may soon be available online as a self-directed course.
